What options for care do you provide?

Valley Oaks Care Center offers short term care and rehab as well as long term care in our skilled nursing facility. At Valley Oaks we also have a dedicated vent unit and a pulmonary rehabilitation program.

We are proud of our professional staff consisting of professional RNs, LPNs and nursing assistants. Every one of our staff members is devoted to assisting residents meet their daily physical, social, and psychological requirements. Valley Oaks Care Center (formerly Nentwick Care Center) has been giving residents the care, dignity and respect they deserve for generations.

How do you figure out my level of care?

Your level of care is determined by our specialized care team, who will review your current health, including your medical history, to learn the best way to take care of you and address all of your needs.

Are transportation services available to residents?

Yes, at specific times there is access to transportation for residents.

How much does it cost to come to Valley Oaks Care Center?

Cost can vary by a lot, depending on the level of care the resident needs and the level of insurance coverage the resident has. Please give us a ring at (330) 385-5001 to learn more about the cost involved.

Is Valley Oaks Care Center the right place for me?

Your first step in determining if Valley Oaks is the right place for you is to take a tour (tours are free!). While you are exploring our scenic grounds, visiting buildings, and speaking with residents, you will also have a chance to meet various team members. Do you offer short term care?

Yes. Short term care is best for those recovering from a hospital stay or in need of help to make the transition back home, or to take care of someone when a family caregiver is not available (for instance, while on vacation). If there are limited recovery choices, or if extra time is necessary to move back home, our professionally trained care team can help. Our care team includes RNs, LPNs, and nursing assistants to help make any necessary transition. Learn more about our short term care.

Are your skilled nursing services available 24 hours a day?

Yes! Valley Oaks offers 24-hour skilled nursing care in our care facility.

Can I bring my pet with me?

Yes, although there may be some limitations involved. Please discuss your pet with our staff when coming in for the free tour.
